Finasteride + Ketoconazole Shampoo: Evidence & Protocol

Bottom line: Combining finasteride with ketoconazole shampoo is a common practice in hair loss dermatology. The drugs act via distinct pathways — finasteride reduces DHT systemically; ketoconazole reduces scalp inflammation and fungal load. Evidence for the combination specifically is limited but biologically plausible, and the safety profile is favorable.
Why Ketoconazole May Help in AGA
Malassezia yeast colonization of the scalp is associated with seborrheic dermatitis and may amplify scalp inflammation that contributes to AGA progression. Ketoconazole reduces Malassezia burden, decreases scalp inflammation, and — in vitro — has weak anti-androgenic properties. A small RCT by Piérard et al. (2002) showed ketoconazole 2% shampoo produced modest hair density improvement compared to unmedicated shampoo in men with AGA.
Practical Protocol
| Drug | Typical use | Application |
|---|---|---|
| Finasteride 1 mg (oral) | Daily | Oral tablet; any time of day |
| Ketoconazole 2% shampoo | 2–3× per week | Leave on scalp 3–5 min before rinsing |
The shampoo does not interfere with finasteride’s pharmacokinetics when used topically at standard frequency. Daily use of ketoconazole shampoo is not typically recommended as it can dry the scalp.
