Finasteride + Low-Level Laser Therapy (LLLT)

Bottom line: Low-level laser therapy (LLLT), also called photobiomodulation (PBMT), is an FDA-cleared treatment for AGA that works via a completely different mechanism than finasteride. The two can be safely combined. Evidence for the combination specifically is limited, but given their distinct mechanisms there is no pharmacological reason for interference, and some studies show additive benefit with LLLT + minoxidil ± finasteride regimens.
How LLLT Works
LLLT devices emit red or near-infrared light (typically 630–670 nm) at low energy levels that penetrate the scalp and are absorbed by mitochondrial cytochrome c oxidase in hair follicle cells. This upregulates ATP production in follicle cells, potentially extending anagen phase and stimulating miniaturized follicles. The mechanism is additive to DHT-reduction by finasteride because it operates at the cellular energy level rather than the hormonal level.

FDA-Cleared Devices
Several LLLT devices have received FDA 510(k) clearance for hair loss treatment. These include laser caps, helmets, and combs from various manufacturers. FDA clearance indicates safety and “substantial equivalence” to predicate devices — it does not constitute full FDA approval of efficacy equivalent to drug trials. Look for devices with 510(k) clearance and at least one published RCT supporting their specific device.
Practical Protocol
- Continue finasteride daily as prescribed — LLLT timing does not interact with oral finasteride pharmacokinetics
- Most cleared LLLT devices specify use 3× per week, 15–30 minutes per session
- Do not apply topical products immediately before LLLT — clean dry scalp maximizes light penetration
- Allow 6–12 months before assessing response
